I recently stopped by Five Guys (because sometimes nothing else will do), and as I was tossing my receipt, I noticed the survey invite. Turns out, if you take a quick customer satisfaction survey, you can enter for a chance to win one of ten $75 Five Guys gift cards each month. So of course, I gave it a try—and it was super easy.

How to Take the Five Guys Guest Survey
To get started, you’ll need a receipt from your most recent visit. There’s a small section on it that invites you to take the survey—along with a few details you’ll need to enter online.
Here’s what to do:
Go to FiveGuys.com/Survey
Enter the store number and date/time from your receipt
Answer questions about your visit—things like how your food tasted, how the staff treated you, and how clean everything was
You will be asked if you would like to enter into the sweepstakes, after you select “YES” you will enter your contact info.
That’s it! The whole thing took me less than five minutes. It’s mobile-friendly too, so I just did it on my phone while waiting for my food to settle.
What’s the Reward?
This isn’t one of those “get a free fry instantly” deals, but it’s still worth doing. When you complete the survey and enter your details, you’ll be added to that month’s sweepstakes drawing. Each month, 10 winners get a $75 Five Guys gift card.
It’s not guaranteed, of course—but hey, your odds are better than doing nothing with that receipt.

- You’ll need to take the survey soon after your visit—so don’t wait too long
- Double-check that your receipt is still legible, since the info is required
- If you win, you’ll be contacted by the info you provide at the end (so use a real email)
My Take
I always appreciate when companies give you something back for your feedback. Sure, it’s a sweepstakes, but it takes less time than checking your socials—and there’s no catch. Five Guys made it simple, and the questions felt relevant. If I end up scoring a gift card, even better!
So if you’ve got a recent receipt, give it a shot. Even if you don’t win, you’ll have taken part in helping improve one of the tastiest burger chains around.

food and service was good- I have eaten at many of your stores at a variety of locations and have always found one thing in common—very loud music and not enjoyable selections- I have asked people if it was just me and every answer was the same, they found music and volume excessive– add to that the employees yelling out order numbers and socializing loudly creates an unpleasant environment. There needs major improvement in these areas.
Prices are a bit high before inflation hit in 2022. But crew was great.Food IS great!
The Coke Original flavor, was out and the mgr came and put in a refill….both were out,(she might have had only one) maybe since they were small concentrate, and the other dispenser was empty, it would have been a good idea to do them “both” at that time. Who knows? Was glad it could be refilled myself. I just assumed the refills were in house for drinks. But one never knows these days.🤗😌

My wife and I felt like a milkshake and after ordering found I left my wallet behind. I told William to never mind, but he said no problem, we can cover it, and he gave us the shakes on the house. What a Great Kind gesture! And the shakes really hit the spot.

Staff was helpful and informative. Food, two patty bacon cheese and flavored milkshake was very tasty and I will return.
